The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Participate in peer chart audits and outside audit preparation.
- Prepare and present training, outreach, and educational sessions and materials.
- Supervise interns in graduate and undergraduate programs.
- Prepare written and statistical technical reports.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.

- Collaborate with supervisor, team members and other Human Services programs and community agencies.
- Make site visits and provide other community interventions as needed.

- Provide supportive counseling and advocacy for clients.

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.

This is a permanent at-will position with the City Manager’s Office reporting to the Director of the Office of Racial Equity. Bilingual in Spanish or Vietnamese is preferable but not required.

Under the direction of the Director of Engineering, this position provides engineering support to NCPA’s Power Generation Service department.  The Power Generation Service Department includes the “State of the Art” Lodi Energy Center (LEC) Combined Cycle Power Plant, our green Geothermal Power Plant, our Hydroelectric Power Plant, and includes our hydrogen fuel source with a mission to provide green and renewable energy to our 16-member agencies in Northern California.

ESSENTIAL DUTIES
Manage and assist with power plant improvement projects, performance monitoring, and other initiatives. Conduct feasibility analysis, and justification, and prepare plans, and budgets.
Lead or assist with Root Cause Analysis (RCA) to investigate incidents that cause “forced outages”, equipment malfunctions, failures, or performance degradation at plants.
Provide administrative and end-user support for NCPA’s Computerized Maintenance Management System (CMMS) and Plant Information (PI Historian). Ensure that users are properly trained and the application is used in an optimal fashion. 
Support compliance with NERC/WECC reliability standards.  Assist in the preparation, review, collection, and organization of compliance evidence and associated procedures.

MINIMUM QUALIFICATIONS
Education 
For all levels, the qualified candidate will have a Bachelor’s degree from an ABET-accredited college or university with a focus on Mechanical or Electrical Engineering.

The actual salary shall be determined by the final candidate’s qualifications and experience. In addition to the starting salary, employees in the Associate Engineer classification shall also receive an approximate five percent (5%) ongoing non-pensionable compensation pay.

The Department is currently recruiting to fill multiple Associate Engineer (Building) positions in the Plan Review Services Division with duties including, but not limited to:

- Advising professional engineers, architects and others concerning code requirements, engineering and planning problems arising in the preliminary design of buildings and structures.

- Examining plans, calculations and specifications for residential, multi-family, commercial and industrial buildings, including complex major structures for structural adequacy, zoning and general compliance with governing laws, codes, ordinances and regulations.

- Making independent engineering analyses on proposed buildings when required to justify the structural safety of design, and applicable electrical and mechanical systems.

- Working closely with and giving information to professional engineers, architects, contractors and the general public regarding building, zoning and general requirements concerning buildings and structures.

- Coordinating effectively with Development Service partners e.g. Planning, Fire Prevention, Public Works, Environmental Services for plan review consistency and streamlining permit process.

- Issuing permits after review of plans.

- Advising building inspectors on structural, fire and life-safety challenges arising in the field during the construction of a building project.

- Instructing inspectors and technicians in the review of simple building plans.

- Conducting safety assessments of older buildings and assisting in enforcing compliance with the City's Disaster Response Plan.

- Performing observational review of critical structural elements of the projects under construction.
