THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. The Psychiatrist I/II will provide medical and psychiatric treatment and diagnostic services to clients in the caseload of Tri-City and provide collaborative work with a multidisciplinary team in a public mental health agency. Provide integrated, effective, and high quality behavioral health care to the residents of Pomona, Claremont and La Verne with moderate to severe functional impairment due to mental illness.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Perform intake and diagnostic interviews on a scheduled and emergent basis.
• Participate in diagnostic staff evaluation of clients.
• Provide medical psychiatric treatment of clients.
• Maintain and disseminate medical records and reports in compliance with Federal, State and Agency regulations, including Health Insurance Portability and Accountability Act (HIPAA) and The Health Information Technology for Economic and Clinical Health Act, Public Law 111-005 (“HITECH Act”).
• Submit required statistical and other requested information on a timely basis.
• Consult and provide information to other agencies that are coordinating their work with that of the behavioral health service agency.
• May serve on Utilization Review Committee, Medication Review Committee, and chair Peer Review Committee as assigned.
• May conduct specialized research, special studies, and services.
• Provide medical consultative services to the behavioral health services staff; lead team conferences and provide clinical direction in unit(s) to which assigned.
• Perform other duties as assigned.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Participate in peer chart audits and outside audit preparation.
- Prepare and present training, outreach, and educational sessions and materials.
- Supervise interns in graduate and undergraduate programs.
- Prepare written and statistical technical reports.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

- Manage a caseload of clients comprised of individuals with complex medical and co-occurring conditions, including SUD, mental illness, or homelessness.
- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.
- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.
- Provide crisis intervention.
- Collaborate with supervisor, team members and other Human Services programs and community agencies.
- Make site visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Provide supportive counseling and advocacy for clients.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Prepare and present training and educational programs and publicity materials.
- Supervise interns in graduate and undergraduate programs placed at HCR.
- Prepare written and statistical technical reports.

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.

THE POSITION:

The City of San Rafael Library & Recreation Department, Child Care Division, is accepting applications for Child Care Instructor I to work in our School-Age Programs.  Under the supervision of the Child Care Director or Head Teacher, you will assist in the planning and implementation of daily activities for the school-age program. 

This position performs the following essential job duties:      

• Assist in planning and implementing age appropriate curriculum, activities, and events.
• Provide continuous supervision to children in day-to-day activities; ensures health and safety of children.

• Prefer possession of a valid California Driver’s License
