The Department also directly offers services to residents throughout their life course from infants to elders. Services offered to families are designed to help them become or remain self-sufficient. The Aging and Family Services (AFS) Division is committed to meeting the needs of a growing senior population and providing a continuum of services to help elders remain independent, safe and in their own homes.

The Position

Under direction, the Case Manager will perform professional and clinical social work with older adults with complex medical conditions and/or families to assist them to obtain health, financial and/or social services.  As a member of the Human Services Department, this position reports to a Clinical Supervisor, and is characterized by the responsibility to provide comprehensive psycho social assessments and develop and implement care management plans. That may focus on connecting to public benefits, budgeting, accessing medical care etc.  The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits to client homes, medical facilities, and other community locations.   

Examples of Responsibilities

- Manage a caseload of clients comprised of individuals with complex medical and co-occurring conditions, including SUD or homelessness.
- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.-

The Position

As an integral team member of the Aging and Family Services Division, the Mental Health Specialist provides support to the mobile mental health team in maintaining wellness of clients stepping down from the senior mobile mental health program.

Examples of Responsibilities

- Work with team members to assist in the monitoring, follow-up and collateral work of assigned caseload.
- Interact with clients, families, and organizations in a variety of settings including: homes, hospitals, doctors’ offices, schools and other city, county, or state agency offices and on the phone.
- Perform human services support activities including referring clients/family members to appropriate community resources according to the client care plan.
- Follow up on client services, schedule meetings and appointments.
- Interview and elicit personal information from clients, and program information from service providers; provide assistance to clients such as completion of forms, clarify documentation discrepancies, and resolve issues related to clients’ needs and requests.
- Convey general information regarding program services and interpret policies and regulations to clients, other agencies, and the community.
- Maintain confidential records, accurate client files, screen and process forms, verify authorizations on documents, and update automated databases.

The Position

This Counselor position is an integral member of the Senior Mobile Mental Health team offering outpatient mental health services to homebound elders in their place of residence. In collaboration with a clinical supervisor, psychiatrist, psychiatric nurse and AFS case managers, the Counselor will provide wrap around services to seniors and their families. The incumbent will interact with seniors and their families by conducting in-home assessments (DSM 5) and reassessments to develop thoughtful and creative care plans.  In addition, the Counselor will provide counseling services to seniors with mental illness and guidance to family members caring for them.

- Receive and review requests for services.
- Conduct intake evaluations to determine type and severity of problems.
- Establish diagnostic and treatment plans.
- Interview clients, relatives and other involved persons to evaluate clients' needs and assess clients condition.

- Perform individual, family and group counseling to enhance mental and emotional health, level of social functioning and independent living for children, adolescents, adults and older adults using a variety of therapeutic and counseling techniques.

THE POSITION:

Under the general supervision of the Youth & Family Services Clinical Supervisor, the Case Manager provides professional and clinical social work with high-risk adolescents, young adults, and/or families to assist them to obtain educational, health, employment and social services; coordinates and tracks all new referrals; maintains accurate case files, tracking systems and prepares reports as directed; participates in department and community meetings as necessary and may provide training and assistance to other staff or volunteers; performs related duties as needed.

ESSENTIAL DUTIES:

Duties may include but are not limited to the following: 

• Manage an active caseload of 20-25 clients.
• Receive, process and track all incoming case management, outreach, and counseling referrals.
• Conduct phone intakes with counseling and case management referrals and provide resources for immediate needs as necessary.
• Conduct comprehensive client assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information as appropriate to develop a case plan.
• Participate in the NHUSD’s Student Attendance Review Board and provide case management services for Union City youth in coordination with the Alameda County Truancy Court.
• Provide referrals for employee readiness programs, assists with employment applications and job readiness skills.
• Develop support systems to meet client needs by identifying and coordinating a variety of available services necessary to obtain family stabilization and self-sufficiency.
• Provide crisis intervention services, supportive counseling and advocacy for clients.
• Conduct home, juvenile detention center and school site visits as needed.
• Monitor and verify services provided to each client on a monthly basis to determine the quality and effectiveness of services provided.
• Prepare case notes and technical reports.
• Develops and implements new processes and procedures as needed.
• Communicate with other service providers, probation, schools, etc. as needed
• Prepare and presents training and educational programs.
• Attend meetings, trainings and workshops.

BACKGROUND

The Fremont Family Resource Center (FRC) is a collaborative effort of over 20 State, County, City and non-profit agencies that support families in a variety of ways.   Services available at the FRC include: adult and youth employment; child care information, referral and subsidies; counseling and case management; housing information; parent support; immigration services; services for the disabled; nutrition services for mothers and children; health insurance counseling and SparkPoint FRC to help families build their financials assets.   The City of Fremont Human Services Department is the lead organization in the FRC collaborative and in SparkPoint FRC.

SPARKPOINT FREMONT FRC

Helping families get ahead financially is one of the core strategies identified by the FRC as an effective means of empowering and strengthening families and children.  In this vein, the FRC is one of 10 Bay Area SparkPoint Centers, and initiative coordinated by United Way of the Bay Area aimed at helping families to reach their financial goals.  The FRC is also a partner in “Earn It! Keep It! Save It! Alameda County, the county wide free tax preparation campaign helping families to claim the EITC.  

THE POSITION

Under the supervision of the SparkPoint Program Coordinator, the VITA Site Coordinator oversees site operations at a Volunteer Income Tax Assistance (VITA) site at the Fremont FRC, and 2 satellite sites located at Tri-Cities One Stop Career Center; and New Haven Adult School in Union City.  The Site Coordinator manages volunteers and works together with the VITA Quality Assurance Coordinator to efile returns in TaxSlayer and sends required reports to UWBA. The Site Coordinator coordinates and works with the Quality Assurance Coordinator, the VITA Coaches and Trainers and Quality Reviewers and FRC Staff to ensure that the site operates according to all guidelines, and that required statistics and surveys relating to the taxpayer information is handled appropriately.

This position is a part-time, temporary assignment.  The work schedule is as follows:
- 20 - 30 hours/week through December 16, 2019,
- 40 hours/week January - April 17, 2020, then
- 20 hours/week through October 2020 with two months off in July and August

Ability to attend monthly UWBA Site Coordinator meetings and trainings in Oakland required.

RESPONSIBILITIES

- Oversee all operational needs of a tax site.
- Recruit volunteers and ensure all volunteers meet the IRS training and documentation requirements according to their volunteer positions.
- Coordinate closely with quality reviewers and coaches and support their efforts to ensure accuracy of returns.
- Work closely with SparkPoint Coordinator to develop seamless connections to other SparkPoint Asset Building programs and services for clients.  
- Coordinate services with Census Fremont project, sharing opportunities for cross training and outreach.  
- Track and report hours for volunteers.  
- Maintain and monitor on-line customer appointment logs.
- Work collaboratively with In-take Sites and UWBA to coordinate the virtual VITA process.  Responsible for assigning returns to tax preparers, tracking return status, contacting customers, and updating and maintaining appropriate log sheets.
- Communicate effectively with site staff and to manage facilities support and scheduling issues
- Ensure timely electronic filing of tax returns and resolution of rejects, working closely with the VITA Quality Assurance Coordinator.   
- Address equipment, software, supply and other site needs in a timely fashion, working together with the IRS and UWBA EIKISI staff.
- Coordinate volunteer hospitality and volunteer recognition event, in collaboration with SparkPoint Coordinator.
- Communicate regularly with volunteers and maintain volunteer database.
- Create marketing materials for the VITA Program, i.e. flyers, articles for newspapers, electronic newsletters, email blasts, etc.
- Execute site procedures according to UWBA Campaign guidelines.
- Manage workflow of tax site: set-up space; facilitate client sign-in and intake; ensure orderly and thorough screening process and reduce wait time.
- Manage supplies and materials, keep files and supplies organized, and order new supplies when necessary. Keep track of site budget and expenses.
- Ensure security of confidential information.
- Ensure proper reporting and record keeping. Collect, compile and submit to the Campaign required statistics and surveys relating to the taxpayer information.
- Serve as the main contact for clients to address questions or concerns about their experience at the tax site. Promptly respond to inquiries from clients or the IRS.
- Other related duties as assigned.

THE POSITION

The City of Fremont’s Human Services Department is currently looking for a temporary employee to serve as Winter Shelter Coordinator. This position is responsible for the daily operations of the shelter and overseeing monitors and volunteers who staff the facility.  This position reports to the Human Services Director, or her designee, and coordinates with the Department’s Executive Assistant regarding scheduling, reporting, and other administrative tasks.

The Coordinator position can work up to 40 hours a week, between the hours of 4:30pm and 7 am.  The Winter Shelter will be open from November 15th through March 15th, to provide a safe and welcoming environment for individuals and families experiencing homelessness. Participants will have access to the shelter from approximately 6:30 p.m. to 7:00 a.m., where they will be provided a warm dinner, restroom facilities, sleeping bags, cots, breakfast, and a to-go lunch.

EXAMPLES OF RESPONSIBILITIES

- This position is responsible for maintaining the health and safety of homeless individuals and families who are living on the streets or in their cars and are in need of emergency shelter.
- Oversee the monitors to ensure that they follow procedures and support them in decision-making, while enforcing rules and procedures with participants.
- Maintain the inventory of supplies to ensure they are adequate for the functioning of the warming shelter.
- Coordinate with City Serve Compassion Network regarding food and site volunteers to ensure timely delivery of meals and adequate volunteer support of shelter functions.
- Responsible for accurate, daily data collection.
- Coordinate medical services with Tri-City Health Services.
- Ensure that monitors complete nighty report of their observations and notes.
- Coordinate with Abode Services regarding outreach workers.
- Work compassionately with those who have difficulty engaging with service providers.
- Maintain a welcoming, safe, and equitable environment for clients, monitors/other staff, and volunteers.
- Effectively resolve conflicts among participants and de-escalate potentially violent situations.
- Coordinate with janitorial and building maintenance to communicate issues and schedule routine cleanings.
- Ensure participant adherence to rules, especially related to behaviors that are unsafe.  If necessary, notify public safety personnel to help “exit” clients with inappropriate or unsafe behaviors.
- Exercise good judgement and be sufficiently confident to call for 911 assistance in case of medical or other emergency.
- Coordinate with Human Services Executive Assistant regarding monitor work schedules to ensure adequate staffing at all times.
- Other related duties as assigned.

POSITION: The City of Fremont opens their Winter Shelter every winter to provide a safe and welcoming environment for individuals and families experiencing homelessness. Participants have access from approximately 6:30 p.m. to 7 a.m., where they will be provided with a warm meal, restroom facilities, sleeping bags, cots, Breakfast, and a to-go bag. The Winter Shelter is open from mid-November to mid-March. The Human Services Department is currently recruiting for Winter Shelter monitors for next season (2019/2020).

The position will work up to 30 hours per week.

THE POSITION

DISTINGUISHING CHARACTERISTICS

Incumbents may advance from the Assistant Civil Engineer to the Associate Civil Engineer level, as recommended by their supervisor and approved by the Director of Public Works, as they learn City and departmental practices and procedures. As knowledge and experience are gained, the work becomes broader in scope and assignments are more varied and are performed under more general supervision.  Advancement to the higher classification requires gaining the knowledge, skill, experience and credentials that meet the qualifications for that class and demonstrating the ability to consistently perform the work of the higher classification.
