San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

Examples of Duties

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

• Actively participate in quality improvement efforts.

• Perform related duties as assigned.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

Licensure

• California Registered Nurse license

• California Board of Registered Nursing (BRN) Nurse Practitioner license

• California BRN Furnishing Number

• DEA Number

For OB-GYN assignments, a Nurse Practitioner license may be substituted with a Nurse Midwife License and at least one certification in women’s health.

The Manager, Track & Structures reports to the Deputy Director of Infrastructure Engineering and is responsible for overseeing railroad track and structures engineering support for the Peninsula Corridor Joint Powers Board (Caltrain)

Essential Functions & Duties:

Essential Functions:

• Acts as agency lead on matters related to track and structures.
• Oversee the track and structures engineering tasks for design/construction of Caltrain Capital and SOGR projects and provide technical support for planning on grade separation projects.
• Ensure the capital and maintenance projects comply with the contract agreement and implement the approved design for compliance with JPB standards and specifications. 
• Lead the advanced technical reviews of projects to assess constructability, examine design details, perform quantity audit, and recommend construction sequencing.
• Establish effective relationships and liaise with Caltrain operation contractors, consultants, internal departments, and other agencies (CPUC, FRA, UPRR, etc.) on railroad track and structures related to influence design and project development.
• Supervise staff.  Hire, mentor and take appropriate corrective and/or disciplinary action.  Ensure EEO policies and procedures are followed. Participate in selection of staff. Coordinate staff training and professional development. Establish performance objectives. Monitor and evaluate employee performance.

Examples of Essential Duties: 

• Plan, develop and implement the group’s work plan, budget, goals, and objectives to be consistent with the Engineering Department’s goals.

San Mateo Medical Center (SMMC) is seeking an experienced Patient Services Specialist (PSS) - Extra-Help.
 
The successful hire will support the clinical team with all registration, eligibility, enrollment, authorization, other administrative tasks, and Admission, Discharge, and Transfer (ADT) activities for SMMC. The PSS serves as the ambassador for SMMC to validate information and educate our patients and guests with the financial process upon arrival to the facility. The PSS verifies all demographic information, validates eligibility, obtain authorizations, collects co-payments, assist with all payor notifications and communication, and enroll patients if needed. The hire will be expected to work holidays and rotating weekends, if needed, to support department needs.
 
The ideal candidate for this position will possess excellent communication skills, including the ability to explain complex medical terminology and medical insurance requirements to individuals with varying levels of medical knowledge. Additionally, they should have strong skills in conflict resolution and problem-solving.

Group Supervisor I ($34.61/hr-$43.24/hr) - Completion of at least 60 semester or 90 quarter units from an accredited college or university. An emphasis in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Group Supervisor II ($39.04/hr-$48.83/hr) - Graduation from an accredited four-year college or university. A degree in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Selected candidates must be available to work flexible shifts, including graveyards, day shift, swing shift, weekends and holidays at any of the Probation Department locations. Selected candidates must also successfully pass a background investigation, including fingerprints, medical examination, psychological evaluation, a truth verification test and be free from any physical, emotional or mental condition which might adversely affect the exercise of the powers of a Peace Officer. Felony and misdemeanors convictions may be disqualifying depending on type, number, severity, and recency. Group Supervisors hired into permanent positions must pass PC 832 peace officer training and complete a 160 hour STC training course within one year of appointment.

There are currently various vacancies in the Probation Department. Depending on qualifications, the vacancy may be filled at the Group Supervisor I or Group Supervisor II level.

Group Supervisors are responsible for:

• Supervising daily activities of youth in confinement.
• Observing and supervising unstable youth.
• Enforcing security and safety measures including searches of youth and their living quarters for contraband.

Qualifications

Group Supervisor I: Completion of at least 60 semester or 90 quarter units from an accredited college or university. An emphasis in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Group Supervisor II: Graduation from an accredited four-year college or university. A degree in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Licensure/Certification:

• Possession of a class C California driver license or equivalent.

Other Requirements:

• Must be at least 21 years of age on or before date of appointment.
• Must be legally authorized to work in the United States by federal law.

• A bachelor’s degree in business administration, public administration, transportation, finance, engineering, computer science, or a related field.
• 3 years of increasing experience in operations, program or project management, or transaction based systems in relevant industries such as tolling or transportation, financial services, technology/software systems, or logistics. Experience must include program or project oversight, vendor or contractor management, system or data analysis, and customer service.

Preferred:
• Experience with public agencies, tolling systems, or technical program management
• Familiarity with electronic payment systems, DMV processes, and vendor coordination
