San Mateo Medical Center (SMMC) is seeking an experienced Patient Services Specialist (PSS) - Extra-Help.
 
The successful hire will support the clinical team with all registration, eligibility, enrollment, authorization, other administrative tasks, and Admission, Discharge, and Transfer (ADT) activities for SMMC. The PSS serves as the ambassador for SMMC to validate information and educate our patients and guests with the financial process upon arrival to the facility. The PSS verifies all demographic information, validates eligibility, obtain authorizations, collects co-payments, assist with all payor notifications and communication, and enroll patients if needed. The hire will be expected to work holidays and rotating weekends, if needed, to support department needs.

San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the unclassified position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.

The current unclassified vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.
