The Position

Under the direction of the Youth and Family Services Administrator, the Senior Program Coordinator plays a unique role overseeing the early childhood mental health program, providing direct counseling services and supervising several staff counselors and interns.

The Senior Program Coordinator typically has specialized experience, training and qualifications that enable the position to work effectively with children 0-8 years, parents and youth caregivers.

The Senior Program Coordinator works closely with the division’s Clinical Supervisors and Quality Assurance Specialist to ensure that clinical services comply with all policies, procedures and legal regulations, and that client services are delivered at the highest standards. The position also coordinates with other key positions in the Human Services Department to help clients have easy and timely access to coordinated services throughout the Human Services Department and/or with other partner agencies.

Examples of Duties

- Oversee and coordinate the direct services under the early childhood mental health program, working closely with the counselors and interns assigned to the program.
- Provide administrative and clinical supervision and consultation to YFS counselors and interns/trainees who are providing direct counseling services to children 0-8 years, parents, and family members, while maintaining close cooperation with the Youth and Family Services Division’s Clinical Supervisors and Quality Assurance Specialist.
- Provide direct counseling and early childhood consultation services to children 0-8 years, parents, and family members.
- Together with the YFS Administrator, understand policies and trends related to the provision of early childhood mental health services; and represent the division at County and/or school district meetings.
- Ensure direct service staff and interns are familiar with and fully utilize client Electronic Health Record System and becomes a key user representative on system implementation team.
- Oversee clinical documentation and compliance with State Medi-Cal documentation standards in partnership with Clinical Supervisors and Quality Assurance Specialist.
- Manage the client intake and referral process for the early childhood mental health program.
- Ensure that staff meet contractual and quality assurance goals and audit requirements.
- In partnership with YFS Administrator, Clinical Supervisors, and the Human Services Department’s Fund Development Officer respond to requests for proposals, develop grant work plans, narratives, and budgets.
- Oversee early childhood grant deliverables and reporting for the division.

DEFINITION: Under general supervision, the Field Monitor provides operational support and supervision of Mill Valley parks, outdoor fields, offsite facilities, and occasionally the Mill Valley Community Center. Responsibilities will include but will not be limited to patrol and observation of outdoor facilities, ensuring compliance with facility regulations and use guidelines, general customer service, light office tasks, and the performance of other related work as required. This is a part-time, year-round position.

EXAMPLES OF DUTIES:

• Monitor user activities in department facilities and enforce all rules, regulations, and safety principles.
• Patrol field, park and offsite facilities to monitor rental schedules and permitted use.
• Act as a liaison to the outdoor recreational community for the Recreation Department.
• Provide quality customer service and information for all Department facilities, classes, programs, and events.
• Maintain appearance and overall environment of facilities by completing daily tasks and cleaning as needed.
• Occasionally assist with the coordination and execution of Department-sponsored outdoor events or classes.
• Maintain records and reports as required.
• Enforce all rules, regulations and safety principals.
• Willingness and ability to work flexible hours including evenings, weekends and holidays.
• Ability to take the lead in completing assignments when working with a partner or on a team.
• Respond professionally to emergencies, provide care as indicated under the scope of training.

DEFINITION: Under general supervision, responsibilities will include but not be limited to light office tasks, customer service, set up and take down of facility equipment and janitorial tasks. Perform related work as required. This is a part-time, year-round position.

• Set up and take down tables, chairs, audio-visual equipment, and other related material and equipment.
• Greet public and provide general information for all Recreation department classes, programs, and events.
• Daily janitorial/maintenance duties such as routine and special cleaning of restrooms, community center rooms (i.e. wipe counters, vacuum floors, sweep walkways, empty trash receptacles, replace paper products and litter pick-up).
• Monitors guest activities in the community center and enforce all rules, regulations and safety principals.
• Assist in opening and closing procedures for the entire Community Center.

The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

Examples of Duties

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.
- Collaborate with team members, Human Service programs, and community agencies.
- Conduct home visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Advocate for clients in accessing services.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Participate in peer chart audits and outside audit preparation.
- Prepare and present training, outreach, and educational sessions and materials.
- Supervise interns in graduate and undergraduate programs.
- Prepare written and statistical technical reports.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Examples of Duties

- Manage a caseload of clients comprised of individuals with complex medical and co-occurring conditions, including SUD, mental illness, or homelessness.
- Conduct comprehensive client assessments to develop appropriate service plans and monitor them closely.
- Assist clients to develop support systems to maintain independent living, self-sufficiency and family stabilization.
- Provide crisis intervention.
- Collaborate with supervisor, team members and other Human Services programs and community agencies.
- Make site visits and provide other community interventions as needed.
- Conduct inter-agency and/or family conferences.
- Provide supportive counseling and advocacy for clients.
- Monitor services provided for each client.
- Maintain timely electronic documentation of services, reporting, and billing.
- Prepare and present training and educational programs and publicity materials.
- Supervise interns in graduate and undergraduate programs placed at HCR.
- Prepare written and statistical technical reports.

DEFINITION: Under general supervision, provide personal fitness training to clients requesting service; maintain personal schedule at the front desk; enforce all rules and regulations pertaining to the safety and well being of all patrons; keep fitness floor tidy and clean. Perform related work as required, may include light cleaning and janitorial.

EXAMPLES OF DUTIES:

• Train clients safely according to current industry standards and training.
• Offer and conduct free fitness floor orientations to new customers during Fitness Floor shifts.
• Learn basic operation instruction for fitness equipment and teach these techniques to visitors. Provide corrective feedback and proper technique.
• Maintain certain class records and reports as required.
• Ensuring that a safe and healthy environment exists for exercising. Monitor guests’ activities in the fitness center.
• Enforce all rules and regulations.
• Recognize and respond professionally to emergencies, provide care as indicated under the scope of training.
• Assume assigned duties connected with care and maintenance of the facility.
• Provide basic maintenance, equipment care.
• Deal tactfully and courteously with the public.
• Establish and maintain effective working relationships with other city employees, other agencies, and the general public.
• Actively attend and participate in required staff training and in-service drills.
• Ability to communicate clearly both orally and in writing.
• The ability to speak English clearly and understandably.
• Perform related duties as required.

TRAINING & EXPERIENCE: Certified Personal Trainer required. Additionally, combination of education, training, and experience, that provides the required knowledge and abilities.

Fitness Floor Shift times: Monday-Friday 3:30-6:30pm

• Clean, sanitize and ensure that fitness equipment is working properly

• Enforce all rules, regulations, and safety principles
• Answer any general questions from patrons and providing information on fitness programs
• Assume assigned duties connected with care and maintenance of facility
• Maintain certain fitness center records and reports as required

• Respond professionally to emergencies
• Perform related duties as required

TRAINING & EXPERIENCE: Any combination of education, training, and experience that provides the required knowledge, abilities and CPR within 3 months of hire.
