San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

Examples of Duties

Duties may include, but are not limited to, the following:

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Outreach to and build long-term, trust-based relationships with vulnerable populations.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Deliver longitudinal care in assigned areas.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

• Counsel families regarding health maintenance and care.

• Coordinate referrals to other health or social agencies.

• Maintain accurate records of assessments, follow-ups, and evaluations.

• Supervise subordinate health workers and participate in meetings.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Process Improvement Manager that plans, oversees, and provides a wide variety of administrative, analytical, and work-flow support on a County-wide level; analyzes practices and procedures and makes recommendations for organizational, operational, policy, and procedural improvements; conducts needs analysis, feasibility studies, and program evaluation for assigned projects and programs; develops, summarizes, and maintains administrative and fiscal records; fosters cooperative working relationships among County departments and acts as liaison with various community, public, and regulatory agencies; and performs related work as required.

There is currently one regular vacancy in the Public Health, Policy and Planning Division, located in San Mateo, but the position will include travel/driving throughout the county to perform duties of the job.

This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics, and will exercise direct and general supervision over assigned professional, technical, and office support staff.

Primary responsibilities will include, but are not limited to, the following:

• Quality assurance and improvement: build and visualize data in management reports and dashboards; convert data to information and use it to guide process improvement.

DISTINGUISHING CHARACTERISTICS

The Assistant Engineer is the entry level class in the professional civil engineering series and is responsible for performing the professional engineering work not requiring registration as a Civil Engineer. This classification is distinguished from the next higher classification of Associate Civil Engineer in that the latter performs the more complex and difficult assignments within the division.

The Associate Engineer is the journey level class in the professional civil engineering series and is responsible for performing the more complex design and construction of capital improvement projects and for reviewing private development projects for design conformance and accuracy. Incumbents perform complex professional engineering assignments requiring considerable knowledge of various aspects of civil engineering including land development, planning, design standards and regulations, construction and maintenance. This classification is distinguished from the next higher classification of Senior Civil Engineer in that the latter performs the most complex and difficult assignments within the division and may exercise direct supervision over assigned project staff.

The Senior Civil Engineer is an advanced journey level registered professional engineer class and exercises full responsibility for planning and organizing a variety of assigned engineering projects and directing the work of assigned staff. Incumbents perform complex professional engineering and program management assignments demanding considerable knowledge of various aspects of engineering and related program requirements including land development, planning, design standards and regulations, construction and maintenance. PHYSICAL, MENTAL AND ENVIRONMENTAL WORKING CONDITIONS

Position requires sitting, standing, walking on level and slippery surfaces, reaching, twisting, turning, kneeling, bending, stooping, squatting, crouching, grasping, crawling and making repetitive hand movement in the performance of daily duties.  The position also requires repetitive hand movement and fine coordination in preparing reports using a computer keyboard.  Additionally, the position requires both near and far vision in reading written reports and work related documents. The Department

The Library and Community Services department is responsible for providing excellent customer service through recreational, educational, and cultural programs for children, adults and seniors. San Mateo County Health's Behavioral Health and Recovery Services Division is seeking a well-qualified individual for the position of Supervising Mental Health Clinician - Adult and Youth Services. Supervising Mental Health Clinicians supervise and coordinate the professional work of mental health professionals, paraprofessionals and interns in San Mateo County and participate in program planning, administration and evaluation; and provide specialized psychiatric casework counseling and therapeutic services to adults and youth in the field.
 
The current vacancy is in Behavioral Health and Recovery Services (BHRS) Crisis and Outreach Team including the Psychiatric Emergency Response Team, the Crisis Response Team (CRT), Healthcare for the Homeless (HCH), Homeless Engagement Assessment Linkage (HEAL) team, and the law enforcement, mental health partnership within multiple cities in the County.

Supervising Mental Health Clinicians are members of the Behavioral Health and Recovery Services Leadership Team and work with collaborative partners on system wide initiatives focused on improving services and service delivery. This position reports directly to a Clinical Services Manager.
 
The Supervising Clinician oversees daily operations and directs a multidisciplinary team providing recovery-based behavioral health services, outreach and linkage for BHRS' crisis and outreach teams. Services provided include: supervising mobile clinicians who are working in tandem with city law enforcement at behavioral health crises and/or with homeless outreach programs throughout the County, supervise the program specialist for the Crisis and Outreach Team, and conduct trainings and presentations to law enforcement and the community.
 
The teams include psychiatrists, social workers, marriage and family therapists, law enforcement officers and community workers who are consumers or family members. The clinical teams may also work with clinical interns.  
 
The ideal candidate will possess:

• Knowledge of system of care principles.

• Knowledge of crisis de-escalation, crisis evaluation, 5150-hold application, and community crisis response.

• Facilitate regular meetings with clinicians and all other systems partners that Crisis and Outreach Team collaborates with.
• Ongoing clinical training for field clinicians and law enforcement including the Quarterly Crisis Intervention trainings.

Qualifications

Licensure/Certification:  
Possession of a valid California license in at least one of the following:  Registered Nurse; Clinical Psychology; Clinical Social Worker; or Marriage, Family and Child Counseling.

Education and Experience: 
Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying. A typical way to qualify is a Master's degree in a mental health specialty, such as social work, nursing, or clinical counseling and two years of professional post-graduate experience in a mental health setting.

Knowledge of: 
Basic program management techniques and practices; principles and practices of supervision and training; human and health services system in the County including private agencies and their community resources; current philosophy, principles and practices for services to clients with serious mental illness; legal and policy aspects of community mental health methods; techniques of family, individual and group counseling and diagnosis; individual, group and family psychodynamics; current principles and practices of psychiatric casework; mental health consultation and education; and problems, needs, attitudes and behavior patterns of patients/clients.

MINIMUM QUALIFICATIONS:

Additionally, applicants must meet the following education requirements to qualify:

Group Supervisor I ($34.61/hr-$43.24/hr) - Completion of at least 60 semester or 90 quarter units from an accredited college or university. An emphasis in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Group Supervisor II ($39.04/hr-$48.83/hr) - Graduation from an accredited four-year college or university. A degree in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Selected candidates must be available to work flexible shifts, including graveyards, day shift, swing shift, weekends and holidays at any of the Probation Department locations. Selected candidates must also successfully pass a background investigation, including fingerprints, medical examination, psychological evaluation, a truth verification test and be free from any physical, emotional or mental condition which might adversely affect the exercise of the powers of a Peace Officer. Felony and misdemeanors convictions may be disqualifying depending on type, number, severity, and recency. Group Supervisors hired into permanent positions must pass PC 832 peace officer training and complete a 160 hour STC training course within one year of appointment.

There are currently various vacancies in the Probation Department. Depending on qualifications, the vacancy may be filled at the Group Supervisor I or Group Supervisor II level.

Examples Of Duties

Group Supervisors are responsible for:

• Supervising daily activities of youth in confinement.
• Observing and supervising unstable youth.
• Enforcing security and safety measures including searches of youth and their living quarters for contraband.
• May perform housekeeping tasks.
• Providing orientation for newly detained youth.
• Explaining and enforcing rules and regulations.
• Providing transportation for youth from the County's detention facilities to other facilities and medical offices.
• Supervising daily activities such as meals, showers, housekeeping, laundry, yard work, recreation, vocational work crews, and visitation.
• Providing individual and group counseling.
• Leading group discussions and facilitating rehabilitation programs.
• Maintaining daily records of observations, incidents and events.
• Logging information, writing reports and summaries of behavior and maintaining records and files.
• Physically restraining assaultive youth and de-escalating violent behavior.
• Maintaining a safe environment for the youth and staff.
• Being a positive role model and authority figure.
• Participate in staff conferences.

Qualifications

Group Supervisor I: Completion of at least 60 semester or 90 quarter units from an accredited college or university. An emphasis in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Group Supervisor II: Graduation from an accredited four-year college or university. A degree in sociology, psychology, corrections, criminal justice, counseling or a closely related social or behavioral science is preferred.

Licensure/Certification:

• Possession of a class C California driver license or equivalent.

Other Requirements:

• Must be at least 21 years of age on or before date of appointment.
• Must be legally authorized to work in the United States by federal law.
• Must not have been convicted of a felony.
• Must successfully pass a background investigation prior to employment.

THE POSITION

Tri-City is looking for one (1) individual to serve as a Psychiatrist I/II for the Adult Outpatient Clinic. The Psychiatrist I/II will provide medical and psychiatric treatment and diagnostic services to clients in the caseload of Tri-City and provide collaborative work with a multidisciplinary team in a public mental health agency. Provide integrated, effective, and high quality behavioral health care to the residents of Pomona, Claremont and La Verne with moderate to severe functional impairment due to mental illness.

• Perform intake and diagnostic interviews on a scheduled and emergent basis.

The City of Hayward is excited to announce its recruitment for the position of Mobile Mental Health Clinician. Under general direction, the Mobile Mental Health Clinician provides field and office based professional behavioral health counseling, psychoeducation, outreach and engagement, and case management services as part of the City’s Hayward Evaluation and Response Teams (HEART) Program improving public safety responses to people experiencing mental illness, substance use disorders and homelessness.

THE PROGRAM

The Hayward Evaluation and Response Teams (HEART) Program is a cross departmental program between the Hayward City Manager’s Office, Police, and Fire Departments and Alameda County Behavioral Health Care Services developed to improve access to medical, mental health, and other support services.  The HEART program aims to achieve two main goals: 1) to reduce the need for police officers to intervene in calls for service involving people experiencing chronic mental illness, substance use disorders, and homelessness and 2) to reduce reliance on emergency services for individuals needing long-term case management and follow-up services.  HEART consists of three distinct response teams: the Mobile Integrated Health Unit (MIHU) made up of Hayward Fire Department Community Paramedics; the Hayward Mobile Evaluation Team (HMET) which pairs a specially trained Hayward Police Officer with an Alameda County behavioral health clinician; and the Case Management and Mental Health Linkages Team (LINK) consisting of Mobile Mental Health Clinicians and a Lead Program Assistant (Case Manager).

THE IDEAL CANDIDATE

 As a member of the LINK team, the ideal candidate has experience providing short term crisis intervention, safety assessments, and other mental health services to adults, youth and children experiencing mental illness, substance use disorders and/or homelessness.  They have the ability to work primarily in the field and have the situational awareness to respond alongside emergency response teams. They are knowledgeable about local community resources and skilled at forming relationships quickly.  
  

Essential Duties

What you will do:

Provides direct casework, individual and group therapy; secures and evaluates medical or psychosocial data such as behavior patterns, use of coping skills and family relationships; analyzes factors which exert an adverse effect upon health issues, treatment and recovery which contribute to or are responsible for behavioral health issues and/or affect the potential for recovery and resiliency. 

Performs field work to proactively serve the unhoused population, persons with mental illness and others in crisis.
 
 Interprets the social aspects of physical or behavioral health issues to clients, relatives, other behavioral health and medical care staff and appropriate persons or agencies.
 
 Provides crisis intervention as needed; stabilizes clients in crisis including brief intervention in clinic; arranges for hospitalization and coordinates treatment elements.
 
Assists clients in understanding clinical health challenges and their reaction to them, accepting the need for treatment, building on strengths and working toward the solution of challenges and stresses the importance of engaging in treatment to increase social functioning, modifying unrealistic attitudes, and strengthening client's effort toward satisfactory adjustments.
 
Establishes and maintains effective relationships with contractors and community organizations and other governmental agencies regarding programs and services; participates in committees, meetings and conferences; provides guidance regarding appropriate social-psychological practices and services.
 
Compiles and prepares reports and documentation on clinical activities; maintains records; prepares periodic reports.
 

Identifies emerging trends related to crimes and issues; develops and implements prevention and intervention program approaches; provides community education on current procedures and practices related to mental health and intervention strategies.
 
Serves as a liaison with law enforcement, school officials, social service agencies, and the general public regarding difficult and/or complex situations related to mental health and social services.
 
Provides support and guidance to Fire and Police First Responders concerning victim advocacy, juvenile and adult crime, domestic violence, chemical dependency, and other services as needed.

License & Certificates:  Possession of a valid professional mental health therapy license issued by the State of California, such as Licensed Clinical Psychologist, Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Associate Social Worker License (ASW), Intern Marriage Family Therapist (IMFT) or Clinical Psychologist or a closely related field is highly desirable.

Possess and maintain a valid California Class C Driver's License.
